Rejections, Denials and Appeals

How to tell the difference and get the claim paid.

Rejected and denied are not the same thing. Treating them as if they were is one of the quietest reasons claims sit unpaid past 90 days. This course teaches you to tell which one you are looking at, what each one needs from you, and how to find money that has already gone invisible inside your own software.

Built around interactive worksheets

Not a PDF workbook you print and never open. These are working pages you use with a claim in front of you, built for this course and not available anywhere else.

Two words that get used as one. They are not.

When a claim comes back unpaid, most offices do the same thing no matter what happened. They resubmit, or they wait. Both can be exactly the wrong move, because the fix depends entirely on where the claim stopped.

Rejected

The claim never reached the payer. It was stopped before anyone adjudicated it, so there is nothing to appeal. It needs to be corrected and sent again, and the clock is still running while it sits.

Denied

The claim reached the payer and was refused. There is a remittance, a reason, and a path forward. Sometimes that path is a correction. Sometimes it is an appeal. Resubmitting the same claim usually just earns a duplicate denial.

Once you can see that difference in the paperwork, a lot of stuck money starts moving again.

The part that makes it stick. You work it, you do not watch it.

Most billing training hands you a video and a printable. You finish the video, you feel informed, and on Monday you are looking at a claim with none of it in your hands.

This course is built the other way around. Every section has a working page you open in your browser, beside your own claim, and fill in as you go. They were built for this material and they exist nowhere else.

Six claims came back

An interactive reference where you pick a claim, read where it stopped, commit to an answer, and only then see why. Two of the six are neither a rejection nor a denial.

Four tabs, worked in order

Each page moves from the anchor, to the rule, to your own claims. You are sorting real work, not answering questions about a video.

Nothing is stored, by design

These pages save nothing anywhere, which is exactly why they are safe to use beside live work. A print button on every page turns your work into a PDF you keep.

A printable version of each

The same work on paper for the people who think better with a pen, ready to take to your own work list.

What is inside

Everything is taught software independent, so it works whether your office runs one practice management system or another. Every example uses invented data. No real patient information appears anywhere.

Who this is for

Front desk staff and billers working United States claims. Practice owners and office managers who handle their own billing and keep finding the same claims on the aging report. Anyone who has ever resubmitted a claim, waited a month, and gotten the same answer back.

Why I teach it this way

I have spent more than 30 years in healthcare, including 21 years as an EMT, and years inside billing software helping practices recover claims they had written off. Almost everything I teach comes from a claim somebody asked me to look at because the money had not come in. Software training teaches where the buttons are. It does not teach why the payer said no, and that judgment is what gets claims paid.

Worth knowing before you buy.

This course teaches United States medical billing. If you bill under another country's system, it will not map to your work.

It is one module of my full course, sold on its own. The full course covers the whole picture, from the front desk to posting payments. Enrollment is open now for the Medical Receptionist and Biller Course. Buying this course does not discount that one.
